Assessment of foot posture in patient with diabetes mellitus using foot posture index

Abstract
Background: Diabetic peripheral neuropathy (DPN) is a common complication of diabetes mellitus leading to foot deformities, ulcers, and amputations. Early detection of structural foot changes is crucial for preventing these complications. Objective: To assess and compare foot posture in diabetic patients with and without peripheral neuropathy using the Foot Posture Index (FPI-6). Methodology: This observational study included 50 type 2 diabetic patients (aged 35-55 years) divided into two groups: with neuropathy (n=25) and without neuropathy (n=25). Foot posture was assessed using FPI-6.Neuropathy was identified using Neuropathy Total Symptom Score-6 (NTSS-6). Results: Patients with DPN showed significant postural deviations, with 40% presenting highly supinated feet compared to only 13% in the non-neuropathic group. Non-neuropathic patients predominantly maintained normal foot posture (47%) versus 26% in the neuropathic group. Conclusion: FPI-6 effectively identifies significant foot posture alterations in diabetic patients with peripheral neuropathy. Routine foot posture assessment should be integrated into diabetic foot screening protocols for early intervention.
